Key Responsibilities:
- Provide clinical guidance through telephonic and electronic communication, utilizing clinical judgment and established triage protocols.
- Collaborate with healthcare providers to ensure optimal patient care.
- Deliver tailored patient education on preventive measures, chronic disease management, and medication adherence.
- Prepare patients for consultations by adhering to established rooming standards.
- Administer prescribed medications and injections in compliance with departmental protocols and regulatory standards.
- Assist healthcare providers during various procedures and examinations.
- Conduct Point of Care Testing, including but not limited to flu, strep, and urine tests.
- Collect and prepare laboratory specimens for analysis.
- Manage prescription refills as directed by the healthcare provider.
- Monitor and track test results, ensuring timely follow-up with patients.
- Maintain cleanliness and organization of examination rooms and medical instruments.
- Engage in care coordination among specialists and facilitate transitions of care.
- Participate in performance improvement initiatives aimed at enhancing patient experience and care quality.
- Actively contribute to daily team meetings and discussions.
Qualifications:
- Completion of an accredited nursing program (LPN or RN).
- Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
- AHA CPR/BLS certification is required within the initial onboarding period.
- Prior experience in a primary or specialty care setting is preferred.
