Key Responsibilities

  • Conduct comprehensive assessments and develop personalized care plans for patients.

Qualifications

  • Graduation from an accredited nursing program.

  • Familiarity with electronic health records; experience with EPIC is preferred.
  • Proven ability to work collaboratively within a healthcare team.
