Key Responsibilities- Perform comprehensive assessments of patients' health needs, including their health status, behaviors, level of function, psychosocial situation, and available support systems.
- Establish care plans in collaboration with the primary care physician and the patient care team.
- Provide health education to patients and their families.
- Identify patients at risk for proactive intervention and develop strategies to address their needs.
- Coordinate care for complex cases or those patients seeing multiple specialists.
- Maintain accurate and up-to-date documentation of patient assessments and care plans.
- Analyze patient care trends and collaborate with the care team to improve overall quality and efficiency of care.
- Associates Degree in Nursing (ADN/ASN)
- Current RN license in the state of practice
- Minimum of two (2) years of clinical experience in an outpatient setting or a minimum of two (2) years of experience coaching individuals with complex and/or chronic conditions
- Bachelors Degree (BS/BA/BSN)
- Prior experience in care coordination in a community setting or primary care practice
- Program development, operations management, and/or supervisory experience
- Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
- Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy.
- Expected to lift and utilize full range of movement to transfer patients.
