We are committed to developing and executing solutions that address the increasing demands of vulnerable children and families both in the U.S. and globally.

The Case Manager for Post Release Services (PRS) plays a crucial role in providing essential support for Unaccompanied Children (UC) and their families. This position may also involve conducting Home Studies (HS) as required. The Case Manager will assist children and families in accessing resources, establishing community connections, and delivering case management services, all while applying trauma-informed care principles throughout the process.

This role requires the ability to operate effectively with moderate supervision, adhering to established procedures and meeting agency, federal, and state regulatory standards.

KEY RESPONSIBILITIES
  • Deliver culturally and linguistically appropriate Post Release Services (PRS) and comprehensive case management support;
  • Manage a caseload in alignment with contract stipulations and agency expectations as outlined by the program supervisor;
  • Maintain effective case capacity and track all caseloads;
  • Conduct home visits as required by contract obligations;
  • Facilitate the reunification process through home study assessments when necessary;
  • Provide resource referrals, community connections, and case management services while employing trauma-informed care principles;
  • Conduct assessments tailored to client needs in collaboration with the program supervisor;
  • Evaluate potential placements for unaccompanied children (UC), including interviews, outreach to families in their home countries, and home study investigations;
  • Complete and document all necessary reports, case notes, and contacts in the designated database in accordance with required timelines;
  • Offer psycho-educational information, referrals, outreach, advocacy, and support to children and families;
  • Provide family stabilization counseling and assist qualified unaccompanied children in accessing services through TVAP;
  • Develop and sustain positive networking relationships with community partners and resources for families;
  • Continuously refer and connect families to available resources;
  • Assess the safety and suitability of placements regularly using child welfare principles and best practices in social work;
  • Serve clients throughout the entire state as assigned;
  • Participate in mandatory training and ongoing educational opportunities to meet all federal, state, and agency requirements;
  • Attend weekly meetings with the supervisor to discuss cases and review case plans;
  • Engage in peer support opportunities within branches for professional growth and sharing of innovative practices;
  • Stay informed about community resources and refer families for assistance as needed;
  • Remain updated on all agency, federal, and state regulatory requirements related to social services;
  • Perform other duties as assigned.
QUALIFICATIONS:
  • Bachelor's degree in Human Services, Social Work, or a related field from an accredited institution, with a minimum of two (2) years of case management experience in child and family services, or at least three (3) years of relevant experience in lieu of education;
  • Bilingual in English and Spanish is required;
  • Proven ability to serve diverse client groups, including refugees and other minority families;
  • Sound judgment and sensitivity to various cultures and the unique experiences of refugees;
  • Familiarity with state, community, and agency resources for victims of abuse;
  • Excellent verbal and written communication skills;
  • Demonstrated clinical, therapeutic, and crisis intervention abilities;
  • Capacity to work independently while maintaining a high level of confidentiality;
  • Proficient computer skills, including knowledge of Microsoft Office suite;
  • Must be at least 21 years old with a valid driver's license and a minimum of 3 years of driving experience in the U.S. to operate a vehicle on behalf of Bethany. Must also pass a Motor Vehicle Records (MVR) check and maintain a reliable vehicle with proof of adequate insurance;
  • Pass a criminal history screening, including checks with state and local child protection agency registries;
  • Commitment to and integration of the agency's Statement of Faith and Mission Statement.
