We are seeking a highly skilled and experienced Pharmacist Sr EX to join our team at InsideHigherEd. As a key member of our Ambulatory Care team, you will be responsible for providing exceptional patient care and leading clinical pharmacy services in our specialty clinics.
Key Responsibilities- Provide patient education on medication regimens and ensure adherence
- Perform clinical monitoring and make drug-related recommendations/interventions based on clinical judgment
- Update patient profiles and other forms of clinical documentation
- Participate in additional clinical and operational projects as assigned by the supervisor
- Perform technical duties of the specialty pharmacy, including proper documentation and maintenance of records and data
- Graduation from an accredited school of pharmacy
- Possession of the legal requirements for the practice of pharmacy as determined by the CA Board of Pharmacy
- Three years of clinical experience as a licensed pharmacist in an Ambulatory Care setting
- Current BLS or ACLS certification
- Excellent planning, organization, and time management skills
- Pharm.D. highly desirable
- Hospital pharmacy residency
