Qualifications:
- Demonstrated commitment to enhancing participant experience, health outcomes, and value.
- Proven ability to cultivate strong relationships with healthcare providers and team members.
- Exceptional clinical, organizational, and communication skills, particularly in settings involving elderly participants and their families.
- Current knowledge of best practices in chronic care and geriatric medicine.
- Possession of an active California physician license without restrictions.
Preferred Qualifications:
- Board Certification in Internal Medicine or Family Practice, with a specialization in Geriatrics.
- Completion of an accredited residency program and a Doctor of Medicine or Doctor of Osteopathic Medicine degree.
