Key Responsibilities:

  • Ensure accurate documentation of diagnoses to reflect the clinical complexity of participants.
  • Analyze clinical and financial performance data to identify improvement opportunities and collaborate with leadership on operational strategies.
  • Act as a liaison with community healthcare providers, specialists, and hospitals to foster strong relationships and support provider recruitment.

Qualifications:
  • Demonstrated commitment to enhancing participant experience, health outcomes, and value.
  • Proven ability to cultivate strong relationships with healthcare providers and team members.
  • Exceptional clinical, organizational, and communication skills, particularly in settings involving elderly participants and their families.
  • Current knowledge of best practices in chronic care and geriatric medicine.
  • Possession of an active California physician license without restrictions.

Preferred Qualifications:
  • Board Certification in Internal Medicine or Family Practice, with a specialization in Geriatrics.
  • Completion of an accredited residency program and a Doctor of Medicine or Doctor of Osteopathic Medicine degree.
  • A deep commitment to supporting the needs of vulnerable senior populations.
