Able Health Services Inc. is currently seeking a skilled Community Services Coordinator to join our team. As a key member of our staff, this individual will play a critical role in providing Targeted Case Management Services to individuals and coordinating activities to plan, explore, access, and maintain supports desired by those we serve.

Job Responsibilities:

The successful candidate will be responsible for:

  • Providing case management services to individuals, including advocacy and coordination of resources and services as needed
  • Assisting with conflict resolution, negotiation, and facilitation within teams
  • Ensuring adequate planning for and development of appropriate Person-Centered Plans (PCPs), in accordance with COMAR and agency policy
  • Maintaining accurate records and documenting consumer-related activities according to established protocols

To be successful in this role, candidates should possess a bachelor's degree in a human service field or equivalent experience. Effective advocacy skills, strong written and oral communication skills, and proficiency in computer systems required to document billable activities are essential. A valid driver's license and access to a reliable automobile are also required. The salary for this position is estimated to be around $50,000-$70,000 per year, depending on location and experience.
