  • Collect and document relevant information related to eligibility for DDA services and make recommendations for priority categorization.
  • Develop and maintain Person-Centered Plans (PCPs) that outline individualized service needs and goals.
  • Collaborate with other team members and external stakeholders to ensure seamless coordination of services and support.
  • Maintain accurate records and submit timely reports as required.

Requirements:

  • Bachelor's degree in a human services field or related field with two years of experience working with individuals with developmental disabilities.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with diverse groups.
  • Strong problem-solving and organizational skills, with the ability to prioritize tasks effectively.
  • Able to work independently and as part of a team, with a commitment to delivering high-quality results.
  • Ability to pass a background check and obtain necessary certifications.

Compensation and Benefits:

  • $45,000 - $55,000 per year, depending on experience.
