Job Summary

Roswell Park Care Network is seeking an Advanced Practice Provider to join our team at Ellis in Schenectady, NY. As a key member of our healthcare team, you will provide high-quality patient care, collaborate with physicians and other healthcare professionals, and contribute to the development of our care network.

Key Responsibilities
  • Assess and manage the physical and psychological status of patients through interview, health history, physical examination, and diagnostic studies.
  • Develop and implement individualized care plans, including diagnostic and therapeutic interventions.
  • Collaborate with physicians and other healthcare professionals to ensure comprehensive patient care.
  • Provide education and counseling to patients and families on health promotion, disease prevention, and management of chronic conditions.
  • Refer patients to other healthcare providers and community resources as needed.
Requirements
  • Current licensure as a registered nurse in the State of New York.
  • Certification as a Nurse Practitioner by the American Nurses Credentialing Center or other national certifying body recognized by the Health Care Financing Administration (HCFA).
  • Basic Life Support (BLS) certification.
  • Bachelor's degree in Physician Assistant studies or completion of a Physician Assistant Certification Program recognized by the New York State Education Department.
