Responsibilities:
  • Conduct thorough care plan assessments to identify client needs and develop personalized plans of care.
  • Develop and implement comprehensive plans of care that address medical, financial, and social needs.
  • Provide ongoing monitoring and support to clients and their families to ensure their well-being and safety.
  • Collaborate with healthcare professionals, community resources, and other stakeholders to ensure seamless care coordination.
  • Document all interactions, assessments, and care plans accurately and efficiently.
Requirements:
  • TN Licensed Registered Nurse (Associates or Higher required, BSN preferred).
  • CCM Certification preferred.
  • Minimum of 3 years of experience in local human service or medical field, as well as community resources regularly utilized by older adults.
  • Reliable transportation and vehicle insurance.
  • Computer proficiency in Microsoft Word, Excel, and Outlook.
