We are seeking a skilled RN Case Manager to join our team at Visiting Nurse Association of Ohio. As a RN Case Manager, you will be responsible for providing comprehensive nursing care to patients in their homes. This is a full-time opportunity with a competitive salary and generous benefits package.
Responsibilities- Utilize the nursing process to assess patient needs and develop individualized care plans.
- Provide direct and indirect care to patients, including medication management and wound care.
- Collaborate with healthcare providers and other team members to ensure seamless care coordination.
- Communicate effectively with patients and families to ensure understanding and compliance with care plans.
- Document patient progress and outcomes in a timely and accurate manner.
- 1-2 years of experience in Med-Surg or hospital setting, preferred.
- Completion of an accredited RN education program with a diploma, Associates or Bachelor's degree. Bachelor's degree in Nursing preferred.
- Registered Nurse, licensed in the State of Ohio.
- Valid driver's license and CPR certification required.
- Bi-lingual a plus.
