Job Summary

We are seeking a highly skilled and compassionate RN Case Manager to join our team in Reno County, KS. As a Field-Based RN Case Manager, you will be responsible for assessing, planning, and implementing care strategies for Medicare and Medicaid members, while proactively engaging them to manage their own health and healthcare.

Key Responsibilities:
  • Assess and plan individualized care strategies for members with complex medical and behavioral needs
  • Utilize strong communication and interpersonal skills to engage members and their families in the care planning process
  • Collaborate with interdisciplinary teams to ensure coordinated care and services
  • Manage transitions of care for members upon discharge from in-patient hospital stays and emergency rooms
  • Provide education and support to members on disease management, medication adherence, and healthy lifestyle choices
  • Identify and initiate referrals for social service programs and community resources
  • Communicate effectively with all stakeholders to ensure quality coordinated care and services
Requirements:
  • Current, unrestricted RN license in the state of Kansas
  • 2+ years of experience working in the community as a RN in acute inpatient care, long-term care, home health, hospice, public health, behavioral health, or assisted living
  • Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook
  • Ability to travel up to 75% of the time with reliable transportation, with a current and non-restricted state of Kansas Driver's License and State-required insurance
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
  • Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
Preferred Qualifications:
  • Bachelor's degree in nursing
  • A background in managing Medicaid and Medicare populations with complex medical and behavioral needs
  • Experience with arranging community resources
  • Experience with electronic charting
