Overview

The Visiting Nurse Association of Ohio is seeking a full-time RN Case Manager for the Med Surg department in Lake County. As a key member of our team, you will provide direct or indirect skilled comprehensive nursing care to patients in their homes.

Responsibilities
  • Utilize the nursing process to assess, plan, and evaluate patient care.
  • Implement prevention, therapeutic, and rehabilitative nursing care to promote patient comfort and safety.
  • Communicate effectively with healthcare providers, patients, and families to ensure seamless care coordination.
  • Observe patient reactions and report significant conditions to the supervisor or physician.
  • Respond to life-saving situations based on nursing standards, policies, and protocols.
Qualifications
  • 1-2 years of experience in Med Surg or hospital setting.
  • Completion of an accredited RN education program with a diploma, Associate's, or Bachelor's degree. Bachelor's degree in Nursing preferred.
  • Registered Nurse, licensed in the State of Ohio.
  • Experience in home health care and 1 year experience in medical-surgical nursing.
  • Valid driver's license and CPR certification required.
  • Bi-lingual a plus.
