About the Role

We are seeking a highly skilled and detail-oriented individual to join our team as a Transfer Center Coordinator at Hackensack Meridian Health. As a key member of our healthcare team, you will play a critical role in facilitating communication between sending and receiving facilities, ensuring seamless patient transfers and exceptional patient care.

Key Responsibilities
  • Communication and Coordination: Facilitate communication between sending and receiving facilities, utilizing telephone, computer software, and other communication tools to ensure accurate and timely information exchange.
  • Documentation and Record-Keeping: Accurately document all communication related to each transfer request in the electronic health record, maintaining a high level of attention to detail and efficiency.
  • Collaboration and Teamwork: Work collaboratively with other healthcare team members to ensure effective communication and patient care.
  • Problem-Solving and Escalation: Identify and escalate barriers and concerns using established policies and procedures, demonstrating expertise in handling high-pressure situations and multiple requests.
  • Technical Skills: Utilize multiple computer applications and telephone systems to facilitate communication and documentation.
Requirements
  • Education and Experience: High school diploma or equivalent, with a strong preference for experience in emergency medical services, computer literacy, and medical terminology.
  • Skills and Abilities: Excellent communication skills, flexibility, adaptability, and ability to multitask in a fast-paced environment.
  • Certifications and Licenses: Emergency Medical Technician (EMT), Paramedic, or AHA Basic Life Support certification preferred.
