About the Role

We are seeking a skilled and compassionate Post-Kidney Transplant Coordinator RN to join our team at Henrico Doctors' Hospital Forest. In this role, you will have the opportunity to make a meaningful impact in the lives of patients and their families.

Job Summary

The Post-Kidney Transplant Coordinator RN is responsible for coordinating and facilitating the pre and peri-operative processes for kidney transplant patients. This includes assessing patient needs, educating patients and families about post-transplant care, and planning follow-up care with the referring physician and other healthcare providers.

  • Coordinating and facilitating the pre and peri-operative processes upon notification of organ availability for a specific patient is required.
  • You must be able to assess transplant patients for specific needs as well as educate the patient and their family regarding their specific needs related to the kidney transplant, including the ongoing follow-up care requirements.
  • You will be responsible for planning follow-up care in collaboration with the referring physician, transplant surgeon and other care providers as well as analyzing pertinent data, communicating findings, documenting information and assuming accountability for decisions and actions.

Salary and Benefits

The estimated salary for this position is $85,000 - $110,000 per year, depending on experience. Additionally, we offer a comprehensive benefits package that includes medical, dental, and vision coverage, as well as 401(k) matching and tuition reimbursement.

Requirements and Qualifications

To be considered for this role, you must have an active RN license in the state of Virginia, a BSN degree preferred, and at least three years of nursing experience with one year in ICU or case management.
