About the Role

We are seeking a dedicated and compassionate Licensed Practical Nurse to join our team at Wellpath. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care to underserved individuals.

Key Responsibilities
  • Assess patients' physical, psychological, and social dimensions to develop individualized treatment plans.
  • Implement treatment programs directed by healthcare practitioners, administering medications and performing diagnostic tests as needed.
  • Collaborate with RNs and staff to ensure effective patient care and respond to health emergencies within standard guidelines.
  • Document nursing encounters using SOAP charting and communicate effectively with nursing staff, physicians, and other relevant personnel.
Qualifications & Requirements
  • Graduate from an accredited school of nursing.
  • Prefer a minimum of one (1) year clinic experience.
  • Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
  • Must be able to obtain and maintain CPR certification.
