Nurse Practitioner or Physician Assistant

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, providing unmatched insight into their overall health and well-being.
About the PositionAs a Home Risk Assessment Specialist, you will work collaboratively with physicians, case managers, and other healthcare professionals to identify and address health risks, provide education and support, and develop personalized care plans. Your expertise and compassion will make a direct impact on the health and well-being of our patients.
Responsibilities- Conduct comprehensive health assessments, including medical history, diagnosis, and treatment, in patients' homes
- Work collaboratively with healthcare teams to identify and address health risks, provide education and support, and develop personalized care plans
- Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits
- Collaborate with primary care physicians to ensure seamless care coordination
- Master's degree in nursing or a related field, or commensurate experience and licensure
- Current RN and NP or PA licensure in the state of practice, with prescription authority or the ability to obtain prescriptive authority
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills
