Nurse Practitioner or Physician Assistant

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, providing unmatched insight into their overall health and well-being.
Key Responsibilities- Conduct thorough adult and geriatric assessments, medical history, diagnosis, and treatment, health education, and case management referrals.
- Work collaboratively with physicians, case managers, social workers, and other healthcare professionals to provide comprehensive care.
- Provide services in a variety of settings, including home visits, skilled facility visits, and telehealth visits.
- Master's degree in nursing or a related field, or commensurate experience and licensure.
- Current RN and NP or PA licensure in the state of practice, with prescription authority or the ability to obtain it.
- Board certification in a Matrix-approved specialty.
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant.
- Strong computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills.
