We are currently in search of a Licensed Practical Nurse (LPN) for Hospice to serve in Eastern Cuyahoga. If you are a qualified LPN eager to make a meaningful impact, this opportunity may be ideal for you.

The LPN for Hospice will play a crucial role in delivering care to patients in alignment with the care plan set forth by the interdisciplinary team. This position may involve working in diverse environments and could necessitate travel.

Key Responsibilities

Primary Duties:
  • Advance the organization's mission by pursuing excellence and nurturing positive relationships with team members
  • Provide direct nursing care under the guidance of an RN Case Manager
  • Engage in patient care planning and coordination
  • Administer prescribed medications within the defined scope of practice
  • Educate patients and caregivers
  • Deliver physical and emotional support to patients and their families
  • Evaluate patient conditions alongside an RN Case Manager
  • Assist with in-home respite and continuous care
  • Support patients with daily living activities and self-care
  • Document care in accordance with established guidelines and standards
  • Train caregivers in patient care techniques
  • Exhibit teamwork and effective communication
  • Comply with the organization's policies regarding attendance and appearance
  • Perform other duties as assigned
Qualifications

Essential Requirements:
  • Graduate from an accredited nursing program and possess a valid LPN license in Ohio
  • Current BLS certification
  • Experience in community health nursing is preferred
  • Proficient computer skills
  • Strong organizational and interpersonal abilities
  • Certification as a Certified Hospice and Palliative Care Nurse (CHPLN) is advantageous
