Nurse Practitioner

NYC Health + Hospitals is seeking a skilled and compassionate Nurse Practitioner to join our team in the Ambulatory Care setting as a Primary Care Provider (PCP). As a Nurse Practitioner in this role, you will be responsible for delivering comprehensive primary healthcare services to patients of all ages within our outpatient clinic.
Job Summary:Under general supervision, serves as a licensed, independent practitioner responsible for planning, coordinating, delivering, and managing comprehensive health care services in a health care setting. Ensures patient and family-centered quality care to improve outcomes and patient experience, in accordance with applicable NYC Health + Hospitals policies and procedures, state and federal rules and regulations, and nursing standards of care.
Key Responsibilities:- Provides continuous comprehensive medical and nursing care, including preventive and supportive health care services to patients and families.
- Manages health problems and coordinates health care for various patient populations.
- Identifies and diagnoses illnesses and physical conditions, and implements therapeutic and corrective measures.
- Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and/or other health care professionals and community agencies, as necessary.
- Evaluates and interprets patient data, in order to plan and implement appropriate nursing interventions.
- Consults with physicians and other health care practitioners in the evaluation of health screening findings to make prospective decisions about treatment that will best support improved outcomes and patient satisfaction.
- Formulates differential diagnosis and the plan of care based on medical history, physical exams, nursing assessment, and other pertinent information.
- Determines effectiveness and modifies as per clinical standards and practice protocols, where needed, to achieve medical and health goals.
- Orders, conducts and interprets diagnostic and laboratory tests.
- Identifies and may prescribe appropriate pharmacological and non-pharmacological interventions and therapies.
- Manages a patient session schedule allowing for both scheduled and non-emergent unscheduled visits; maintains a work schedule that ensures availability and continuity of care; ensures optimal availability and utilization of appropriate resources.
- Provides education, guidance, and direction to nursing staff and other members of the health care team to ensure quality and safe medical and nursing care is provided.
- Provides education and counseling to patients and families regarding health status, treatment and progress, medication management, disease prevention, and health promotion, records, and maintains a system of relevant documentation.
- A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED).
- Current certificate as a Nurse Practitioner issued by the NYSED.
- Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
