Home Health Nurse Opportunity

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at Inova Health System. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement personalized care plans.

Responsibilities:

  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans to meet patients' unique needs
  • Provide skilled nursing care, including wound care, medication management, and education
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions
  • Document patient progress and outcomes in a timely and accurate manner

Requirements:

  • Current licensure as a Registered Nurse in Vermont
  • Minimum of one year of recent, verifiable clinical experience
  • Prior home care experience preferred, but not required
  • Ability to work independently and manage time effectively
  • Strong interpersonal and communication skills
