Job Overview:
The Patient Care Coordinator is responsible for overseeing the coordination of clinical services for patients transitioning from hospital care to home or hospice settings.
This role involves promoting, educating, and coordinating services as patients move from hospital to home healthcare or hospice care.
Qualifications:
Preferred candidates will hold a Bachelor's degree in nursing and have graduated from a CCNE or NLN accredited nursing program. A minimum of 2 years of experience in home health or hospice care is preferred, along with a solid understanding of community resources.
Licensure:
A valid Registered Nurse license in South Dakota is required, and licensure in Minnesota may be necessary depending on the service area. If required, licensure must be obtained within 90 days of starting the position.
Integrated Care Management Model Certification is preferred.
This position also requires a valid driver's license and eligibility for Avera Health's automobile liability insurance.
Work Schedule:
Monday to Friday, 8:00 AM to 5:00 PM; 80 hours biweekly
On-call duties every 10th weekend.
Compensation:
The hourly wage for this position is competitive and based on experience, starting at $44.25 per hour.
