Job Summary

Pathways, Inc. is seeking a skilled Social Worker/Case Manager to support clients in their recovery from chronic and severe mental illness. The ideal candidate will have a strong understanding of case management principles and be able to work independently to assess clients' strengths and needs.

Key Responsibilities
  • Assess clients' strengths and needs to formulate a client-centered goal/treatment plan
  • Prepare quarterly treatment plans for clients, including assessment of functioning in residential, psychiatric/mental, social/vocational, financial, and money management areas
  • Provide individual counseling and referrals for group/family counseling as needed
  • Collaborate with community resources and referral sources to coordinate additional needed services for clients
  • Provide assistance or arrange for assistance with IADL skills, obtaining or managing federal, state, and/or local welfare benefits, making and keeping medical and/or psychiatric appointments, and mediating interpersonal problems
  • Maintain client records in our electronic health record system, including complete progress notes, treatment plans, discharge summaries, and other related documents
Requirements
  • Master's degree in a mental health field with practice experience
  • Two years' experience working with people with mental illness preferred
  • Demonstrated ability to communicate effectively both orally and in writing, strong interpersonal skills, and time management
  • Valid driver's license and use of organization car during normal business hours
