MDS and RN Care Management Lead

POSITION SUMMARY
The MDS / RN Care Coordinator plays a pivotal role in managing the RAI process, ensuring compliance with both federal and state regulations, and working closely with nursing personnel to execute individualized care plans. This full-time role is under the supervision of the facility administrator.
KEY RESPONSIBILITIES
- Oversee the RAI process and guarantee the timely completion of MDS, CAAs, and care plans
- Facilitate care plan meetings with residents and their families
- Ensure care plans are current and accurately maintained
- Review MDS documentation for precision and identify educational trends
- Monitor documentation for residents under Medicare and managed care
- Engage in the Quality Improvement program
- Provide guidance on the financial aspects of RUGS, PPS, and case mix rates
- Stay informed on current regulations through ongoing training and seminars
- RN degree from an accredited institution
- AANAC certification is required
- Current CPR certification
- A minimum of 2-4 years of experience in MDS and long-term care
- Familiarity with Point Click Care is preferred
