Occupational Therapist PRN-OT Home Health Specialist

Pinnacle Home Care is seeking a dedicated and compassionate Occupational Therapist to join our team of skilled home healthcare providers. As a PRN Occupational Therapist, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities:- Evaluate patients' functional status and develop personalized treatment plans to improve their independence and quality of life.
- Conduct thorough assessments of patients' homes to identify potential hazards and barriers to independent living.
- Develop and implement treatment programs to address patients' physical, cognitive, and emotional needs.
- Communicate effectively with patients, families, and healthcare professionals to ensure coordinated and comprehensive care.
- Supervise certified occupational therapy assistants and participate in discharge planning to ensure seamless transitions for patients.
- Master's degree in Occupational Therapy from an accredited program.
- Current licensure to practice as an Occupational Therapist in the State of Florida.
- Two years of clinical experience as an Occupational Therapist.
- Current BLS certification.
- Excellent communication and organizational skills.
