Healthcare Case Manager

We are seeking a skilled Registered Nurse to join our Home Health and Hospice team as a Case Manager. As a key member of our care team, you will be responsible for providing management and delivery of patient care plans in a collaborative environment.

Responsibilities- Lead and collaborate with the care team to develop and implement patient care plans
- Provide hands-on nursing care to patients in Winterhaven and surrounding areas
- Maintain accurate records and documentation of patient care
- RN license in the state of practice
- Graduate from an approved school of practical nursing
- At least one year of RN experience and clinical competence to deliver quality patient care
- A competitive estimated salary of $65,000 - $85,000 per year, depending on experience
