Position Overview:

The Clinical Care Nurse Coordinator is responsible for delivering safe and effective clinical care across various patient demographics, including children, adults, and seniors. This role requires advanced nursing skills to ensure optimal patient outcomes, effective communication, and operational efficiency. The coordinator will also engage in nursing research and assist in the education of staff and students.


Key Responsibilities:
  • Deliver direct patient care approximately 50% of the time
  • Manage all facets of the patient care process, ensuring accurate documentation of care and outcomes
  • Participate in regular team meetings and collaborate with healthcare providers in care planning
  • Act as a nursing liaison with physicians and community resources
  • Promote treatment advancements and expand referral networks through community engagement
  • Serve as a resource for staff regarding evidence-based practices
  • Contribute to the development of nursing policies and training materials
  • Oversee management of supplies and equipment within the patient care environment
  • Prepare operational reports as needed
  • Address clinical issues and lead performance improvement initiatives
  • Provide coaching and performance feedback to team members
  • Coordinate patient care conferences as necessary
  • Ensure timely completion of quality control monitors
  • Undertake additional responsibilities as assigned

Qualifications:

Current nursing licensure in Texas is required, with specialty certification preferred.


Education:

A Bachelor's degree in nursing with three years of nursing experience, or an Associate's degree with five years of experience, or a three-year nursing diploma with four years of experience.


Experience:

A minimum of three years of clinical nursing experience is required, with recent experience in the specialty area preferred.


Physical Requirements:

Must be able to exert up to 50 pounds of force occasionally, 20 pounds frequently, and 10 pounds constantly to move objects.


Security Sensitive:

This position may involve responsibilities that are security sensitive and subject to specific regulations.


Residency Requirement:

Employees must reside and work in Texas.
