Hospice RN On-Call Part-Time

To achieve this, we maintain low patient caseloads and never require our staff to be on call. Schedule: On-call every other weekend, 8am-8pm

Responsibilities:
- Triaging after-hours patient and family phone calls, determining the need for services, and delegating nursing visits as needed
- Collecting data on physical, psychological, social, and spiritual factors impacting patients' and families' needs, and initiating interventions by the interdisciplinary team as necessary
- Communicating with attending physicians, other team members, and agencies to coordinate optimal care
- Performing clinical pronouncements of death and providing support to families
- Maintaining accurate clinical records through timely preparation of care plans and clinical notes
Requirements:
- Graduation from an accredited registered nursing program
- Current licensure to practice as a Registered Nurse in the state
- Minimum two years of varied work experience as a professional nurse
- Demonstrated skill in nursing practices and clinical judgments
- Ability to cope with family emotional stress and tolerance of individual lifestyles
- Current CPR Certification
- Valid driver's license and proof of insurance
