As a Transfer Center RN Coordinator, your passion for enhancing patient experiences will be integral to our mission.
In this role, you will:
- Oversee the coordination of direct admissions and hospital transfers to appropriate facilities within the HCA network.
- Manage ambulance transfers and collaborate with post-acute care providers.
- Utilize a customer-focused approach in a transfer and transport call center, leveraging an electronic database for service profiles and clinical protocols.
- Effectively manage all hospital transfers while consulting with on-call physicians regarding patient care.
- Work collaboratively with HCA facilities to coordinate patient movements and bed assignments.
- Document and track all transfers and transports, ensuring compliance with EMTALA regulations.
- Apply nursing expertise to gather clinical data for appropriate patient placement and transfer facilitation.
- Maintain communication with all stakeholders, including physicians and referring facilities.
- Utilize problem-solving skills to address patient and physician needs effectively.
- Assist in data collection and analysis to improve transfer protocols and procedures.
- Exhibit strong customer relationship skills through effective communication with various stakeholders.
- Graduate from an accredited nursing program.
- Current nursing license in the state of Texas.
- A minimum of 5 years of acute care RN experience.
- BSN or a Bachelor's degree in nursing.
- 2 years of experience in Critical Care nursing, including ED, ICU, or as a House Supervisor.
