We are seeking a highly skilled Clinical Pharmacist to join our team at Cooperman Barnabas Medical Center. As a Clinical Pharmacist, you will play a critical role in providing pharmaceutical care to patients and contributing to the development of our pharmacy services.

Responsibilities:
  • Provide focused pharmaceutical care through inpatient rounding and assume the role of drug information provider to the Internal Medicine teaching service.
  • Provide ambulatory care diabetes services one day per week in the Internal Medicine Faculty Practice.
  • Provide cross-coverage, patient counseling, and assume the role of drug information provider for other clinical pharmacy specialty services, including but not limited to: Renal Transplant, Perioperative services, and Geriatrics.
  • Enhance professional growth and development through participation in educational programs, current literature, workshops, in-service meetings, etc.
  • Monitor specific medication prescribing in accordance with established drug usage criteria, prior to dispensing, as defined by the Pharmacy & Therapeutics Committee.
  • Provide educational rotation for both PGY1 and PGY2 residents.
  • Facilitate PGY1 and PGY2 resident research projects.
  • Provide consults and educate patients regarding their medications for acute disease state management and chronic conditions.
  • Utilize the medication screening process to ensure medication orders are complete, appropriate, within legal parameters, and prioritized.
  • Document and provide analytical support for all clinical activities, interventions, medication errors, adverse drug reactions according to criteria set forth by the Department of Pharmacy.
  • Participate in Cooperman Barnabas Medical Center Diabetes Stewardship Committee.
  • Participate in the RWJBarnabas Health Ambulatory Care Corporate Collaborative.
