Licensed Practical Nurse (LPN)

Purpose/Summary Statement:


The primary objective of this role is to monitor and deliver nursing care to residents within a skilled nursing environment under the guidance of a Registered Nurse.

This position is accountable for ensuring seamless care for residents across shifts by providing direct nursing services and overseeing the activities of Certified Nursing Assistants and support staff.

Engages in educational activities for residents and their families while maintaining the role of resident advocate, emphasizing the Mission and Approach of the organization.

Essential Tasks and Functions:
Conducts assessments of residents upon admission and routinely thereafter, including when health conditions fluctuate. Accurately documents findings, including Minimum Data Sets, under the supervision of a Registered Nurse.

Evaluates the resident's health status and nursing requirements, determining suitable nursing interventions.

Possesses the ability to effectively assess and reassess pain levels, employing appropriate pain management strategies. Provides education to residents and families regarding pain control.

Records alterations in residents' health status and nursing interventions in progress notes and care plans.

Capable of performing waived testing (point-of-care testing) in accordance with Clinical Laboratory and resident care unit protocols.

Interprets results from waived tests and takes necessary actions based on findings.

Administers medications while adhering to the five (5) rights of medication administration, minimizing the risk of medication errors.


Delivers comprehensive resident care in a manner that prioritizes resident safety and reduces the potential for medical errors.

Ensures compliance with current documentation standards and policies.

Demonstrates proficiency in utilizing the organization's electronic health records system.

Alerts physicians to changes in residents' conditions and ensures appropriate follow-up actions are taken.

Timely executes physician orders.

Follows up on information provided during shift reports, as well as concerns raised by residents or their families.

Possesses knowledge of HIPAA regulations.

Implements individualized care plans for residents based on assessments and objectives established by the interdisciplinary care team.

Treats residents and their families with dignity and respect, addressing their psychosocial, cultural, ethnic, and spiritual needs.

Supervises other healthcare team members as necessary to fulfill the established care plan.

Exhibits competence in technical and manual skills in accordance with the scope of practice for
LPNs/LVNs.

Acts as a resident advocate by safeguarding their rights.

Engages with residents, families, colleagues, managers, and other departmental personnel in a compassionate, knowledgeable, and friendly manner.

Consistently involves residents and families in the planning of care, treatment, educational needs, and discharge processes.
