About the Role

CAMBA is seeking a highly motivated and compassionate Case Manager to join our team. As a Case Manager, you will play a critical role in providing support and services to individuals with mental health needs. Your primary responsibility will be to work with clients to develop and implement individualized service plans, connect them with community resources, and provide ongoing support and advocacy.

Key Responsibilities
  • Conduct comprehensive assessments and develop individualized service plans for clients
  • Connect clients with community resources, such as mental health services, housing, and employment opportunities
  • Provide ongoing support and advocacy for clients, including accompanying them to appointments and providing transportation
  • Monitor client progress and make adjustments to service plans as needed
  • Collaborate with other service providers and community agencies to ensure comprehensive support for clients
Requirements
  • Bachelor's degree in social work, psychology, or a related field
  • 1-2 years of experience working with individuals with mental health needs
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable transportation
