About the Role

CAMBA is seeking a skilled Case Manager to join our team in the Office of Mental Health. As a Case Manager, you will play a critical role in providing comprehensive support services to individuals with mental health needs.

Key Responsibilities
  • Conduct intakes, assessments, and service plans to ensure clients receive the necessary support
  • Connect clients with medical, mental health, and substance use treatment services
  • Monitor clients' progress and document via progress notes
  • Function as a client liaison and advocate with outside organizations
  • Escort clients to appointments and assist with benefit applications
  • Collect and maintain client files and charts
Requirements
  • Bachelor's or Associate's degree with one year of equivalent experience, or two years of equivalent experience with sufficient education and technical expertise
  • OMH fingerprinting and relevant experience working with individuals with mental health needs
  • Good written and verbal communication skills, as well as computer literacy in Microsoft Office Suite
