Avera is seeking a skilled Social Worker to join our multidisciplinary team, dedicated to delivering exceptional patient care and outcomes. As a key member of our healthcare team, you will provide medical social casework to patients and families, addressing their psychosocial and continuum of care needs.

Key Responsibilities:

  • Assist patients and families in meeting their psychosocial and continuum of care needs
  • Provide direct care services, consultation, education, policy and program planning, continuous quality improvement tasks, advocacy, and liaison with the community
  • Manage high-risk and problem-prone referrals
  • Supervise and peer social work consultation

Requirements:

  • Master's degree in Social Work
  • Current Social Work license in the state of South Dakota
  • Valid driver's license and insurability by Avera Health's automobile liability insurance carrier

Work Schedule: Monday-Friday, 8:00am-4:30pm; 80hrs/2wks

Pay: $43.50 per hour, with opportunities for career growth and advancement
