Weekend Role Summary:

The Licensed Practical Nurse (LPN) Personal Care Unit Coordinator collaborates with patients, families, care teams, payers, and external organizations to ensure the highest possible standards of physical, mental, and psychosocial well-being. This role also facilitates a secure transition from the facility to subsequent care providers or settings, adhering to all relevant laws, regulations, and organizational standards.


Qualifications and Licensure:

  • Completion of an accredited LPN program
  • Active and valid LPN/LVN license in the applicable State, maintained in good standing throughout employment.
  • Preferred: One (1) year of experience in geriatric nursing
  • CPR certification required upon hire or to be obtained during orientation, with ongoing maintenance of certification throughout employment.

  • Possess advanced knowledge in the nursing field
  • Make independent decisions when necessary
  • Understand relevant practices, procedures, laws, regulations, and guidelines governing operations in a post-acute care environment
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures
  • Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation
  • Maintain professional relationships with colleagues, vendors, and others
  • Ensure confidentiality of all proprietary and sensitive information
  • Adhere to company policies, including those related to harassment and compliance
  • Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training

Core Duties:

  • Document patient information accurately and promptly
  • Manage and coordinate patient care through effective interpersonal communication, enabling patients to achieve optimal physical, mental, and psychosocial well-being
  • Prepare and administer medications as prescribed by physicians
  • Take a leadership role in emergency situations concerning patient and staff safety
  • Collaborate with Registered Nurses to coordinate patient care plans and services
  • Provide exceptional customer service and maintain a positive demeanor towards patients
  • Assist in patient evacuation procedures when necessary
  • Demonstrate reliable and consistent attendance
  • Utilize reasoning skills and sound judgment in all situations
  • Communicate effectively and work collaboratively within an interdisciplinary team
  • Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving throughout working hours
