The Licensed Practical Nurse (LPN) serving as the Personal Care Unit Coordinator collaborates closely with patients, their families, the healthcare team, insurance providers, and external organizations to ensure the highest possible standards of physical, mental, and emotional health. This role is essential in facilitating a smooth transition from our facility to subsequent care providers or settings, adhering to all relevant laws, regulations, and standards set forth by Life Care.

Preferred qualifications include at least one (1) year of experience in geriatric nursing.


  • In-depth knowledge of nursing practices and standards
  • Familiarity with regulations and guidelines governing operations in post-acute care environments
  • Ability to implement and interpret departmental programs, objectives, policies, and procedures
  • Competency in all relevant areas, including patient rights, safety, and sanitation protocols
  • Commitment to maintaining confidentiality of sensitive information
  • Adherence to company policies, including those related to harassment and compliance
  • Demonstration of integrity and professionalism by following Life Care's Code of Conduct and completing required compliance training

Responsibilities include managing and coordinating patient care through effective interpersonal communication, enabling patients to achieve or maintain optimal physical, mental, and emotional well-being.


  • Provide leadership during emergency situations to ensure the safety of patients and staff
  • Collaborate with Registered Nurses to coordinate patient care plans and services
  • Proficiently read, write, speak, and understand the English language
