Job Summary

We are seeking a skilled MDS Coordinator to join our team on a part-time basis. The successful candidate will be responsible for coordinating, managing, and monitoring the written plan of care for each resident, ensuring that their needs are accurately addressed and goals are achieved.

Key Responsibilities
  • Stay up-to-date with current federal and state regulations, as well as professional standards, to ensure compliance and best practices.
  • Collaborate with the MDS team to develop comprehensive care plans that accurately reflect the needs and goals of each resident.
  • Coordinate and manage the written plan of care for each resident, identifying their needs and goals, and ensuring timely and appropriate nursing or clinical involvement.
  • Monitor resident status changes and ensure that necessary adjustments are made to the care plan.
  • Ensure that MDS and support documentation are accurate and meet regulatory and auditor requirements.
  • Perform regular audits of documentation to ensure accuracy and completeness.
  • Assist with the discharge process and ensure a smooth transition for residents.
  • Complete administrative tasks, such as forms and reports, in a timely and accurate manner.
  • Maintain established infection control and standard precaution practices when providing care.
  • Ensure the comfort, privacy, and dignity of residents are maintained at all times.
  • Communicate effectively and tactfully with residents, visitors, families, peers, and supervisors.
  • Attend and participate in departmental meetings and in-services as directed.
Requirements
  • Current State License as a Nurse
  • CPR Certification
  • Knowledge and training in all aspects of the MDS process
Preferred Qualifications
  • One year of experience as a Nurse in a long-term care setting
