Bilingual Community Outreach Coordinator

Target Areas:
Candidates from Hartford, Windsor Locks, Manchester, Norwich, Middletown, Windham, Waterbury, Torrington, and Danbury, CT are encouraged to apply.
Work Schedule:
Monday to Friday, 9 AM to 5 PM, with the expectation of flexibility to accommodate the needs of families served.
The Outreach Care Specialist is responsible for providing individual and family support to ensure that members are connected to essential community services, resources, and care coordination.
Collaboration with ICC staff, families, consumers, community partners, stakeholders, and service providers is crucial to ensure that the appropriate services are accessible to designated members.
The primary objective is to enhance independence and quality of life by assessing individual needs and facilitating access to suitable community services and supports.
Key Responsibilities:
1. Coordinate follow-up care plan needs for members by scheduling appointments or enrolling them in relevant programs.
2. Assess member adherence to medical treatment plans through telephone outreach or on-site visits.
3. Identify barriers to compliance and coordinate solutions.
4. Recognize opportunities impacting quality goals and suggest process enhancements.
5. Recommend modifications to treatment plans and determine the necessity for additional services in collaboration with case management and providers.
6. Facilitate identification and referral to local, state, or federally funded programs.
7. Educate members on strategies to mitigate health risks.
8. Prepare documentation to report on case and compliance updates.
9. Establish and maintain relationships with agencies as outlined in relevant contracts.
10. Perform other duties as assigned.
Minimum Qualifications:
A high school diploma and a minimum of one year of relevant experience are required; or a combination of education and experience that provides an equivalent background.
Preferred Skills and Experience:
1. Familiarity with community resources in the area.
2. Bilingual candidates (Spanish) are preferred.
3. A BS/BA degree in a related field is advantageous.
4. Experience with electronic health records (EHR) systems is beneficial.
For URAC accredited areas, associates in this role are expected to possess strong oral, written, and interpersonal communication skills, along with problem-solving, facilitation, and analytical skills.
