Role Overview:

The Quality Improvement Nurse Coordinator plays a crucial role in overseeing the coordination, implementation, and enhancement of core measure indicators. This position is essential for addressing patient complaints and grievances in line with CMS guidelines.

Key responsibilities include:

  • Engaging in patient education and collaborating with physicians and clinicians on treatment plans.
  • Developing and implementing tools specific to disease management for core measure diagnoses.
  • Conducting clinical documentation, data abstraction, and monitoring of core measure data.
  • Acting as a liaison among physicians, nursing staff, administration, patients, and external organizations regarding compliance and patient safety initiatives.
  • Managing quality improvement efforts, including data management, analysis, and strategy development.

A thorough understanding of core measures, LeapFrog, CMS Star ratings, Just Culture, and quality improvement principles is vital. The ability to foster strong working relationships with healthcare team members is essential for achieving optimal quality scores.

Qualifications:

  • 1-2 years of experience in a management role is preferred.
  • Exceptional interpersonal skills to effectively engage with hospital staff and physicians.
