About the Role:We are looking for a dedicated and compassionate Occupational Therapist to provide high-quality care to our patients in their homes. As an Occupational Therapist with LHC Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities:- Develop and implement individualized therapy plans to help patients achieve their goals and improve their functional abilities.
- Conduct thorough assessments and evaluations to identify patients' needs and develop effective treatment plans.
- Provide one-on-one therapy sessions to patients in their homes, using evidence-based techniques and strategies.
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
- Document patient progress and maintain accurate records of treatment plans and outcomes.
- Current Occupational Therapy licensure in the state of Arizona.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle.
