LHC Group is seeking a skilled Occupational Therapist to join our team in El Mirage, Arizona.
About the JobWe are looking for a dedicated and compassionate Occupational Therapist to provide high-quality care to our patients in their homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities- Provide occupational therapy services to patients in their homes, as outlined in their individualized plans of care.
- Evaluate patients' functional status and occupational therapy needs, and develop and implement plans to address these needs.
- Consult with physicians and other healthcare professionals to ensure comprehensive care.
- Document patient progress and communicate with the healthcare team to ensure seamless care.
- Visit patients in their homes, as needed, to provide ongoing care and support.
- Current Occupational Therapy licensure in the state of Arizona.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle.
