Job Summary

Key Responsibilities
  • Communication and Support: Establish and maintain open communication with participants and their support teams, utilizing phone calls, emails, and other communication channels as needed.
  • Service Coordination: Review and analyze individual participants' services, identifying areas of over- or under-utilization, and communicating with participants and support teams to address these issues.
  • Documentation and Record-Keeping: Maintain accurate and timely documentation of participant services, communicating with support teams to obtain updates, corrections, or delayed documentation.
Requirements
  • Education and Experience: A Bachelor's degree or Master's degree in Social Work is preferred. A college degree in Human Services or a related field will be considered. A high school diploma or equivalent is required. Experience providing OPWDD-funded services will be considered in lieu of higher education.
  • Language Skills: Fluency and proficiency in a second language, preferably Spanish, and English is required for the position.
  • Skills and Abilities: Strong customer service and computer skills, ability to multitask and prioritize, knowledge of Microsoft Excel, Word, and Outlook, excellent verbal and written communication skills, strong organizational skills and attention to detail, and case management experience preferred.
Benefits
  • Flexible Work Schedule: 37.5 hour work week, 7.5 hour work day, flexible work schedule menu to include the ability to work a four-day work week, a four-day work week every other week, or a typical five-day work schedule.
  • Comprehensive Benefits Package: 16 annual vacation days, 12 annual sick days, and 13 annual paid holidays, 401K with up to 10% employer investment, heavily subsidized health, vision, and dental insurance, growth advancement opportunities, and work/home balance.
  • Sign-on Bonus: Candidates with one year of Self-Direction Fiscal Intermediary specific experience on the date of hire will be eligible for a sign-on bonus of $3500, paid in increments of $700 every six months of successful employment.
