Job Summary

We are seeking a skilled Clinical Social Worker to join our team at Corewell Health. As a Clinical Social Worker, you will play a vital role in providing psychosocial care to patients and their families, working collaboratively with an interdisciplinary team to address complex needs.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient needs and develop effective care plans.
  • Collaborate with healthcare teams to provide consultation and resources, ensuring seamless transitions of care.
  • Develop and maintain knowledge of community resources, providing patients and families with access to necessary support.
  • Participate in departmental committees and projects, contributing to the advancement of psychosocial care.
  • Stay up-to-date with federal and state regulations, ensuring compliance in role.
Inpatient Psychiatric Duties

As a Clinical Social Worker, you will have the opportunity to work with inpatients and partial program patients, providing daily group therapy sessions and conducting utilization reviews. You will also be responsible for discharge planning and utilization approvals for post-inpatient care.

Requirements
  • Master's degree in social work.
  • Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.
  • State of Michigan Social Work License. LMSW expected within 5 years of employment.
  • Demonstrate knowledge of growth and development and dynamics of behavior for age-related groups.
  • Excellent oral and written communication skills.
  • Proficient in Microsoft Word and ability to adapt to technical demands of electronic communication.
  • Ability to work independently and contribute positively to team performance.
  • Ability to manage, prioritize multiple situations and tasks while remaining calm.
