Job SummaryWe are seeking a skilled Nurse Practitioner to join our team as a PRN (as-needed) provider. As a member of our team, you will conduct comprehensive health assessments, develop personalized care plans, and collaborate with healthcare professionals to ensure seamless care coordination.
Responsibilities- Conduct in-home health assessments, reviewing medical history, medication adherence, and social environment
- Develop and implement personalized care plans to address identified health risks and needs
- Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure comprehensive care coordination
- Provide education and support to patients and their families to promote healthy behaviors and disease management
- Document all assessments, care plans, and interactions with patients and healthcare professionals in accordance with Matrix Medical Network guidelines
- Masters degree in Nursing or related field, or equivalent experience and licensure
- Current RN and NP licensure in state of practice, with prescription authority or ability to obtain
- Board certification in a Matrix-approved specialty
- Minimum 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
- Excellent verbal and written communication skills
