Job Summary

We are seeking a highly skilled and compassionate Family Nurse Practitioner to join our team at LifeLong Medical Care. As a Family Nurse Practitioner, you will provide high-quality, patient-centered care to our diverse patient population in a community-based setting.

Responsibilities
  • Provide preventive and primary health services to patients in a Family Practice Community Health Clinic setting.
  • Act as an advocate for clients and consult with other health team members, including case managers.
  • Maintain accurate and up-to-date client records necessary for clinic reporting and billing.
  • Perform history and physical examinations, assess clinical status, and establish and maintain medical records.
  • Order and/or perform laboratory and screening tests, request outside consultations and evaluations as needed.
  • Develop and implement a program of care, formulate problem lists, and recommend treatment and medication.
  • Provide education, verbal and written, as well as counseling to patients, families, and caregivers.
  • Recognize and evaluate situations requiring immediate attention of a physician and institute treatment procedures essential for patient life.
  • Support and participate in triage functions, both by phone and walk-in.
Qualifications
  • Commitment to providing primary care services for the underserved, with demonstrated ability and sensitivity in working with diverse populations.
  • Ability to work independently, make sound judgments within established guidelines, and establish effective working relations with staff, clinical providers, managers, and external agencies.
  • Excellent interpersonal, verbal, and written skills, with the ability to work effectively with people from diverse backgrounds and be culturally sensitive.
  • Strong organizational, administrative, multi-tasking, prioritization, and problem-solving skills.
  • Ability to work effectively under pressure in a positive, friendly manner and be flexible and adaptive to change.
Requirements
  • Current California NP or PA license.
  • Training and experience in geriatrics, internal medicine, and/or family practice, with board eligibility or certification required.
  • Interest in working with a diverse patient population in a community-based organization and sensitivity to their needs and health problems.
  • Experience with electronic health records.
