About the Role

This is an exciting opportunity for a skilled professional to join our interdisciplinary team as a Clinical Social Worker I. In this role, you will collaborate with other healthcare professionals to assess patient needs and develop personalized treatment plans that promote positive outcomes.

You will work closely with patients and their families to identify social, emotional, and environmental challenges that may be impacting their mental health. Using your expertise and knowledge of psychosocial assessments, you will help develop and implement effective treatment plans that address these challenges.

In addition to working directly with patients, you will also contribute to the development of the team's approach to delivering care and support. Your input and expertise will help shape the services we offer and ensure that they are tailored to meet the unique needs of each patient.

Key Responsibilities
  1. Conduct thorough psychosocial assessments to identify patient strengths and challenges.
  2. Collaborate with other healthcare professionals to develop and implement individualized treatment plans.
  3. Monitor patient progress and adjust treatment plans as needed to ensure optimal outcomes.
Requirements
  • Bachelor's or Master's degree in Social Work (MSW) required.
  • Licensed Clinical Social Worker (LCSW) certification preferred but not required.
  • Experience working in mental health settings, preferably in clinical or therapeutic roles.
