About the Opportunity

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Rutherford, New Jersey.

This is a unique opportunity to work with adult and geriatric clients in their homes, providing rehabilitative care and skilled nursing services.

Responsibilities
  • Make home visits to clients in designated geographic territories.
  • Perform diagnostic tests and measurements, such as mobility and range of motion assessments.
  • Implement integumentary protective interventions to prevent wound formation and promote wound repair.
  • Develop and implement individualized care plans, including manual therapeutic exercises and gait training.
  • Continually assess and revise the care plan, participating in multidisciplinary care planning as needed.
  • Educate and instruct clients and family members on rehabilitative care and activities to promote independent living.
  • Accurately document observations, interventions, and evaluations in a state-of-the-art touch pad tablet.
Qualifications
  • A current NJ Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
