Job Title: Physical Therapist for Home Health

VNA Health Group is seeking a skilled Physical Therapist to join our team in Rutherford, New Jersey.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our patients in their homes. As a Physical Therapist with VNA Health Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Assess and evaluate patients for physical therapy needs and develop individualized plans of care.
  • Provide skilled physical therapy services to patients in their homes, including exercises, stretches, and other interventions to improve mobility and function.
  • Document patient progress and outcomes, and update care plans as needed.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Provide education and support to patients and their families to promote independence and self-care.
Requirements:
  • Physical Therapy licensure in New Jersey required.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills and ability to use electronic medical records.
  • Excellent communication and interpersonal skills.
