Job Title: Senior Case Manager

Job Summary:

We are seeking a highly skilled and experienced Senior Case Manager to join our team at Community Renewal Team, Inc. The successful candidate will be responsible for providing case management services to customers, coordinating supervision, intake, assessments, referral, and discharge of customers, and daily monitoring of customer contacts/activities.

Key Responsibilities:

  • Provide case management services to customers, including coordination of supervision, intake, assessments, referral, and discharge.
  • Monitor customer contacts/activities and provide daily reports.
  • Coordinate referrals to CRT programs and initiatives, services, entitlement programs, networks throughout the community, and maintain relationships with other community action agencies.
  • Develop and implement Individualized Service Plans (ISPs) for each customer.
  • Monitor and document customer progress and adherence to ISPs.
  • Provide reports to referring agencies, including progress reports, discharge summaries, aftercare plans, and special reports.
  • Represent CRT at community meetings and outreach events.
  • Perform other duties as assigned.

Requirements:

  • Bachelor's degree in social work, human services, or a related field.
  • Minimum of two years of experience providing direct services to customers and staff supervision in social services.
  • Strong communication and interpersonal skills.
  • Ability to work in a fast-paced environment and manage time effectively.
  • Knowledge of human services, criminal justice, court systems, and substance abuse.
  • Specialized training in HIV/AIDS, drug and alcohol abuse, veterans, and/or counseling, domestic violence, and/or any other applicable training related to the field of work.

Preferred Qualifications:

  • Bilingual in Spanish.
  • Valid driver's license and agency vehicle.
  • Pre-employment physical and TB test.
