Job Summary

We are seeking a skilled Care Coordinator to join our team at Arkansas Hospice, Inc.

The estimated annual salary for this role is $63,000-$85,000 based on location and experience.

Key Responsibilities:

  • Provide high-quality nursing care to patients in their homes, focusing on symptom management and pain control.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, documenting all interactions and observations.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Maintain accurate and detailed records of patient interactions, including medication administration and treatment plans.

Requirements:

  • Graduate of an accredited LPN program.
  • Valid LPN licensure in the state of Arkansas.
  • Minimum one year of experience in hospice or home health care.
  • Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Demonstrated knowledge of palliative care principles and practices.
  • Ability to prioritize tasks, manage time efficiently, and maintain confidentiality.
